To become a B Beautiful client, please email or call us with your following information:

  • Full Name, as shown on your Driver’s License.

  • Date of Birth

  • The type of service you are coming in for

  • Phone number, a current and working number.

  • E-mail address that you can access to receive registration confirmation.

Once our staff enters your data, you will get an e-mail from PatientAlly to complete your registration. Please fill out all the necessary consent forms before your appointment at B Beautiful Med Spa.
